[國家一級運(yùn)動員]參賽運(yùn)動員保證書
本人對參加此次賽做以下保證：
1、 我保證：遵守比賽組委會所有規(guī)則和服從一切裁決。
2、 我保證對自己的參賽器材、裝備及個(gè)人所有物品負(fù)有全部責(zé)任。
3、我對在連續(xù)而激烈的比賽中可能發(fā)生的危險(xiǎn)已有充分的認(rèn)識，我保證本人的身體健康狀況能夠承受本次比賽，并具備近期健康檢查證明書。
4、我保證：本人在比賽中因受傷、遭遇意外事故或發(fā)生疾病時(shí)，對受到的急救處理方法及結(jié)果不提出異議。
5、我本人、我的家屬、親戚和保護(hù)人已充分理解本次比賽的內(nèi)容和意義，并認(rèn)可我本人參賽。在比賽之前我已作了人身保險(xiǎn)。因此，我保證：我自己、親屬及遺囑執(zhí)行人、財(cái)產(chǎn)繼承人，對本人在比賽中或參加由本次比賽委會組織的與比賽有關(guān)的活動時(shí)，不論什么原因所致的傷、病、亡或出現(xiàn)的后遺癥等，不對比賽組委會提出任何起訴。所造成的損失由保險(xiǎn)公司和本人承擔(dān)，比賽組委會不再提供因此而造成的損失費(fèi)。
6、我保證：允許比賽組織者自由使用和制作與比賽有關(guān)的我的名字、照片等。并承認(rèn)與本次比賽有關(guān)的新聞、廣播、電視等商業(yè)宣傳報(bào)道。
7、我保證：由于自然災(zāi)害等客觀原因造成比賽取消或變更比賽時(shí)間、內(nèi)容所引起的有關(guān)費(fèi)用問題，服從比賽組委會的決定；由于我個(gè)人的情況而不能參加比賽的，不要求退還參賽時(shí)所交納的各種費(fèi)用。
8、我確認(rèn)：對參賽所進(jìn)行的上述保證已充分理解，并承認(rèn)本保證書解釋權(quán)屬比賽組委會。本人自愿簽字。

i guarantee the following during my participation in this triathlon competttion:

3. ihavefullyunderstoodthedangerwhichmayariseduringthecontinuousandstrenuouscompetition guaranteethatmyhealthconditionisfitforthecompetitionandipossessarecenthealthcertificate.
4. iguaranteenottoraiseanyobjectiontothewayofemergencytreatmentanditsresult if i am injured,illorhaveanunexpectedaccidentduringthecompetttion.
5. i myself, myfamily members, relativesandmyguardianhavefullyunderstoodthecontentandsignificanceofthiscompetitionandhaveapprovedmyparticipationinthecomptition.i have had mypersonalinsuranceforthecompetition. therefore, i guaranteethatimyself, myfamilymembers, executor of testament, propertyadministratorandsoccessorswillnotbringasuitagainsttheorganizingcommtteeofthecompetitionformyinjury,illness, deathandsequelanomatterwhatthecausemay be, duringmyparticipmoninthecompetitionandintheactivmesrelatedtothecompetition. theinsurancecompanywillberesponsiblefortheloss. no damages willbeofferedbytheorganizingcommitteeofthecompetition.
6. iguaranteethattheorganizeeofthecompetitioncanusemynameandthephotoofmefreelyinrelationtothecompetition. irecognizethecommercializationofthenewsreport, radiobroadcastandtelevisionbroadcastinrelationtothecompetition.
7 .i guarantee that i shallacceptthedecisionmadebytheorganizingcommitteeofthecompetitionastothecostwhicharisesfromthecancellationofthecompetitionorthechangesofcompetitiontimeandcontentduetoobjectivecause,suchasnaturalcalanityowingtopersonalconditonwhichpreventsmefromtaking partinthecompetition,norefundwillbedemandedbymeonthevariousihavepaidforthecompetition.
